Estudos de patologia em pacientes com epilepsia trazem informações úteis para compreender a fisiopatologia de várias formas de epilepsia, bem como aspectos relacionados ao tratamento e ao prognóstico a longo prazo. Esses estudos são usualmente restritos a espécimes cirúrgicos obtidos de pacientes com epilepsias focais refratárias. Portanto, a maioria diz respeito à epilepsia do lobo temporal (ELT) com esclerose mesial temporal (EMT) e às malformações do desenvolvimento cortical (MDC). Desse modo, fornecem informações sobre um grupo selecionado de pacientes e sobre regiões restritas do cérebro. Raramente são realizados estudos post mortem do cérebro inteiro em pacientes com epilepsia; entretanto, eles podem prover informações relevantes sobre patologia cerebral, permitindo a análise de áreas além da zona epileptogênica putativa. Neste artigo, foram revisados estudos de patologia feitos em pacientes com epilepsia, com ênfase nos achados neuropatológicos em ELT com EMT e MDC. Foram revisados, ainda, dados de estudos post mortem e discutiu-se a importância da realização desses estudos em populações de pessoas com epilepsia.

Pathology studies in epilepsy patients bring useful information for comprehending the physiopathology of various forms of epilepsy, as well as aspects related to response to treatment and long-term prognosis. These studies are usually restricted to surgical specimens obtained from patients with refractory focal epilepsies. Therefore, most of them pertain to temporal lobe epilepsy (TLE) with mesial temporal sclerosis (MTS) and malformations of cortical development (MCD), thus providing information of a selected group of patients and restricted regions of the brain. Postmortem whole brain studies are rarely performed in epilepsy patients, however they may provide extensive information on brain pathology, allowing the analysis of areas beyond the putative epileptogenic zone. In this article, we reviewed pathology studies performed in epilepsy patients with emphasis on neuropathological findings in TLE with MTS and MCD. Furthermore, we reviewed data from postmortem studies and discussed the importance of performing these studies in epilepsy populations.

OBJETIVO: Analisar retrospectivamente uma série de pacientes com epilepsia do lobo temporal (ELT) e esclerose mesial temporal (EMT), bem como correlacionar os padrões de esclerose hipocampal com os dados clínicos e o prognóstico cirúrgico. MÉTODOS: Foram incluídos neste estudo 66 pacientes com ELT refratária a tratamento medicamentoso e com EMT unilateral submetidos à lobectomia temporal anterior. A análise neuropatológica quantitativa foi realizada em seções hipocampais imunomarcadas com NeuN. Dados clínicos e resultados do acompanhamento pós-cirúrgico foram revisados. RESULTADOS: Ocorrência de evento precipitante inicial e melhor controle de crises após a cirurgia (i.e. classe 1 de Engel) foram associados aos padrões clássico (EMT tipo 1a) e severo (EMT tipo 1b) de esclerose hipocampal. CONCLUSÃO: A análise quantitativa do padrão de perda neuronal do hipocampo é capaz de predizer o prognóstico cirúrgico em pacientes com ELT-EMT.

OBJECTIVE: To analyze retrospectively a series of patients with temporal lobe epilepsy (TLE) and mesial temporal sclerosis (MTS), and the association of patterns of hippocampal sclerosis with clinical data and surgical prognosis. METHOD: Sixty-six patients with medically refractory TLE with unilateral MTS after anterior temporal lobectomy were included. Quantitative neuropathological evaluation was performed on NeuN-stained hippocampal sections. Patient's clinical data and surgical outcome were reviewed. RESULTS: Occurrence of initial precipitating insult (IPI), as well as better postoperative seizure control (i.e. Engel class 1), were associated with classical and severe patterns of hippocampal sclerosis (MTS type 1a and 1b, respectively). CONCLUSION: Quantitative evaluation of hippocampal neuronal loss patterns predicts surgical outcome in patients with TLE-MTS.

OBJETIVO: No presente trabalho avaliamos o impacto da presença de transtorno psiquiátrico pré-cirúrgico sobre o prognóstico cirúrgico em pacientes com epilepsia do lobo temporal e esclerose mesial temporal (ELT-EMT). METODOLOGIA: Analisamos, retrospectivamente, os dados de 115 pacientes com ELT-EMT (65 mulheres, 56,5%) tratados cirurgicamente (corticoamigdalohipocampectomia). As avaliações psiquiátricas foram feitas de acordo com os critérios DSM-IV. O prognóstico favorável foi definido como ausência de crises desde a cirurgia (Engel IA). RESULTADOS: Dos 115 pacientes tratados, 45 (42,6%) tiveram prognóstico favorável e 47 (40,8%) apresentavam transtorno psiquiátrico pré-operatório. A presença de depressão (OR=5,11; p=0,004) foi associada ao prognóstico cirúrgico desfavorável. CONCLUSÃO: A presença de depressão durante a avaliação psiquiátrica pré-operatória é um fator preditivo de prognóstico desfavorável em pacientes com ELT-EMT.

OBJECTIVES: This study aims at verifying the impact of pre-surgical PD on seizure outcome in patients with refractory temporal lobe epilepsy and mesial temporal sclerosis (TLE-MTS). METHODS: After previous consent, retrospective data from 115 surgically treated (corticoamygdalohyppocampectomy) TLE-MTS patients (65 females; 56.5%) were analyzed. Psychiatric evaluations were performed through DSM-IV criteria. Engel IA was established as a favorable prognosis. RESULTS: Forty-five patients (41.6%) were classified as Engel IA, while 47 (40.8%) presented pre-surgical PD. Depression (OR=5.11; p=0.004) appeared as a risk factor associated to a non-favorable seizure outcome. CONCLUSION: In patients with refractory TLE-MTS, the presence of depression predicts an unfavorable outcome.

Este estudo foi delineado para investigar a presença de relação entre a intensidade de sinal em FLAIR e níveis de citocinas, óxido nítrico (NO) e glutamato no hipocampo de pacientes com epilepsia do lobo temporal refratária, associada com esclerose mesial (TLE-MTS). Vinte e nove pacientes foram selecionados através de análise clínica e de ressonância magnética (RM) que foram submetidos a cortico-amigdalo-hipocampectomia para o controle das crises. Os níveis de glutamato foram avaliados por HPLC, as citocinas TNFα e IL1β por ELISA e os níveis de NO via NO system. Avaliamos também por imuno-histoquímica a expressão de IL1β e CD45 em tecidos controles e com esclerose. Tecido controle foi obtido após autópsia de indivíduos mortos sem disfunções inflamatórias e neurológicas (n=10). A concentração de glutamato se mostrou reduzida no tecido TLE-MTS (p<0,001) sugerindo aumento na liberação desse neurotransmissor. TNFα e IL1β também apresentaram níveis elevados no hipocampo dos pacientes (p<0,05), demonstrando um processo inflamatório crônico. Houve uma correlação linear positiva entre a intensidade do sinal em FLAIR e os níveis de NO e IL1β. Em contraste, uma correlação linear negativa foi encontrada entre a intensidade do sinal em FLAIR e níveis de glutamato no hipocampo com esclerose. Infiltrado linfocitário hipocampal também foi visualizado pela imuno-marcação com CD45 em pacientes com TLE-MTS. Esses dados mostraram uma associação entre alteração de sinal na RM e marcadores inflamatórios em pacientes com TLE-MTS.

We investigated a relationship between the FLAIR signal found in mesial temporal sclerosis (MTS) and inflammation. Twenty nine patients were selected through clinical and MRI analysis and submitted to cortico-amygdalo-hippocampectomy to seizure control. Glutamate, TNFα, IL1, nitric oxide (NO) levels and immunostaining against IL1β and CD45 was performed. Control tissues (n=10) were obtained after autopsy of patients without neurological disorders. The glutamate was decreased in the temporal lobe epilepsy (TLE) -MTS group (p<0.001), suggesting increased release of this neurotransmitter. The IL1β and TNFα were increased in the hippocampus (p<0.05) demonstrating an active inflammatory process. A positive linear correlation between FLAIR signal and NO and IL1β levels and a negative linear correlation between FLAIR signal and glutamate concentration was found. Lymphocytes infiltrates were present in hippocampi of TLE patients. These data showed an association between hippocampal signal alteration and increased inflammatory markers in TLE-MTS.

INTRODUÇÃO: Na abordagem de pacientes com epilepsia, particularmente aqueles com crises refratárias ao tratamento clínico, raramente os aspectos relacionados ao suporte familiar desse pacientes são contemplados. Neste artigo nós discutimos conceitos fundamentais como família e suporte familiar e a importância desses conceitos no tratamento multidisciplinar de pessoas com epilepsia. METODOLOGIA: As escalas para mensuração do suporte familiar são discutidas, destacando-se os principais aspectos abordados em cada uma das escalas. CONCLUSÃO: O uso rotineiro dessas escalas permitiria uma análise objetiva do funcionamento e suporte familiar de pacientes com epilepsia, devendo por isso ser objeto de futuros estudos, que enriqueceriam a compreensão do impacto das epilepsias sobre a família, bem como possibilitariam a intervenção precoce nas famílias disfuncionais, com melhora da qualidade de vida.

INTRODUCTION: In the approach of patients with epilepsy, particularly those with clinically refractory seizures, aspects related to the family support of such patients are seldom contemplated. In this article we discuss fundamental concepts such as family and family support, as well as the importance of these concepts in the multidisciplinary treatment of persons with epilepsy. METHODS: The scales that are used for measuring of family support are discussed, with emphasis on the main aspects of each scale. CONCLUSION: Routine use of these scales would allow for an objective analysis of family functioning and support in patients with epilepsy, and thus should be object of future studies, which would enrich the understanding of the impact of epilepsy on families, and would also permit early interventions in dysfunctional families, leading to quality of life improvement.

OBJETIVO: Realizar a adaptação transcultural do instrumento para avaliação da qualidade de vida de indivíduos com epilepsia "Quality of Life in Epilepsy-89" (QOLIE-89) para a população brasileira. METODOLOGIA: Dois tradutores bilíngües fizeram a tradução do instrumento original para o português. A seguir, dois professores de inglês nativos e bilíngües ficaram encarregados da retro-tradução. Na segunda fase uma equipe multidisciplinar "Comitê de Juízes" julgou as diversas versões obtidas quanto à clareza, compreensibilidade, manutenção do conceito explorado em cada questão no instrumento original e sua adequação de sentido para nossa população. No terceiro momento, 21 pacientes do ambulatório da Unidade de Pesquisa e Tratamento das Epilepsias (UNIPETE) responderam ao questionário. RESULTADOS: Quinze pacientes (71.4%) eram do sexo feminino, com idade entre 15 e 49 anos. Oito (38.0%) haviam cursado até o ensino fundamental. O tempo médio de aplicação foi de 45 minutos. As dificuldades de compreensão ocorreram apenas nos pacientes que cursaram até a 8ª série. Na versão final em português sofreram modificações as questões 14, 16, 19, 21 e 85. CONCLUSÃO: A versão em português do QOLIE-89 foi facilmente compreendida, sendo mínimas as modificações realizadas no processo de adaptação cultural do instrumento.

PURPOSE: To describe the cross-cultural adaptation of the instrument "Quality of Life in Epilepsy-89 (QOLIE89)" for Brazil. METHODS: Two independent bilingual specialists translated the QOLIE-89 to Portuguese- Brazilian language, who, after discussion, arrived at a consensus version. Later, two independent translators native in the target language and fluent in Portuguese translated this consensus version back into English. The second step was to compare the back-translation with the source version by a Multidisciplinary Committee. The new version was evaluated in order to preserve conceptual equivalence of the source and final versions, adjustment of cultural words and context, clearly content, and fully comprehensibility. In the third step, a final version was tested in a pilot study including twenty-one outpatients with confirmed epilepsy diagnosis in treatment at UNIPETE. RESULTS: Fifteen (71.4%) subjects were women, with 15 to 49 years. Eight (38.0%) had attended elementary school. The patients required about 45 minutes to complete the questionnaire. In the final Portuguese version of the QOLIE-89, the questions 14, 16, 19, 21 and 85 were considered difficult to understand and were modified. CONCLUSION: The Portuguese-Brazilian version of the QOLIE-89 was easy to understand for most of the patients, after minimal modifications.

OBJETIVO: Este estudo teve como objetivo avaliar a qualidade de vida (QV) em três diferentes grupos de epilepsia e verificar a esfera percebida como de maior impacto na vida diária. METODOLOGIA: A amostra foi composta por 57 pacientes com epilepsias focais e generalizadas do Hospital São Paulo, Universidade Federal de São Paulo, divididos em três grupos, epilepsias do lobo temporal (ELT), extratemporais (Extra-ELT) e generalizadas idiopáticas (EGI). Os pacientes responderam a um questionário preliminar para identificar a percepção sobre os aspectos mais comprometidos em suas vidas. A QV foi avaliada por meio da versão brasileira do Quality of Life in Epilepsy Inventory 31 (QOLIE-31). A correlação dos domínios do QOLIE-31 com a duração da epilepsia e freqüência de crises foi definida pela inspeção dos gráficos de dispersão e pela correlação de Pearson e de Spearman. Foram considerados significantes os valores de p < 0,05. RESULTADOS: Dificuldades nas relações interpessoais, familiares e sociais foram apontadas como a esfera de maior impacto relacionado à epilepsia, citada por 13 (22,81%) pacientes. O QOLIE-31 mostrou resultado semelhante nos três diferentes tipos de epilepsia, com exceção do domínio Funcionamento Cognitivo. Os escores deste domínio foram significativamente menores (p = 0,01) no grupo com Extra-ELT (38,4) do que nos grupos ELT (52,6) e EGI (62,6). A duração da epilepsia não influenciou na QOL nesta amostra, porém foi observada uma correlação estatística significante entre a freqüência de crises e os domínios Efeitos da Medicação (p = 0,0476, alfa = 0,05) e Preocupação com Crises (p = 0,0463, alfa = 0,05). A freqüência de crises mostrou ainda uma diferença estatisticamente significante (p = 0.007) no grupo com Extra-ELT, que apresentou mais crises/paciente, quando comparado aos demais grupos. CONCLUSÕES: Os pacientes identificaram as relações interpessoais, familiares e sociais como sendo a área mais afetada pela epilepsia. O domínio Funcionamento Cognitivo mostrou-se como fator determinante na QOL do grupo Extra-ELT. A freqüência de crises influenciou a QOL nos domínios Preocupação com Crises e Efeitos da Medicação nos três grupos.

PURPOSE: This study aimed to evaluate the quality of life (QOL) and verify the domains of greater impact in patients with focal and generalized epilepsies. METHODS: The sample, composed by 57 subjects from Hospital São Paulo da Universidade Federal de São Paulo, was divided into 3 groups, temporal lobe epilepsies (TLE), extra-temporal epilepsies (Extra-TLE) and idiopathic generalized epilepsy (IGE). They answered a preliminary self-reported questionnaire to identify the perception of the most impaired aspects in their lives. The QOL was evaluated through the validated Brazilian version of the Quality of Life Epilepsy Inventory 31 (QOLIE-31). The correlation of the QOLIE-31 domains with epilepsy duration and seizure frequency was defined by dispersion graphics and also Pearson’s and Spearman’s correlation. RESULTS: The most frequently identified impact of epilepsy was related to interpersonal, familial and social relationships mentioned by 13 (22.81%) patients. The seizure frequency per patient in Extra-TLE group was significantly greater (p = 0.007) than in the other groups. The Cognition Functioning scores were lower for the Extra-TLE group (38.4) when compared with TLE (52.6) and IGE (62.6) (p = 0.01). The correlation between epilepsy duration and QOLIE-31 domains did not demonstrate statistical significance; however, seizure frequency was correlated with Seizures Worry (p = 0.0463, alpha = 0.05) and Medication Effects (p = 0.0476, alpha = 0.05) domains. CONCLUSIONS: 1) Interpersonal, familial, and social relationships were the dimension which most impacted daily life; 2) Cognition domain in Extra-TLE group showed the worst scores; 3) QOL scores were similar in the three groups for the majority of the QOLIE-31 domains; 4) The seizure frequency in the Extra-TLE group was significantly greater; 5) Seizure frequency was associated with worse QOLIE-31 scores in the domains Seizure Worry and Medication Effects.

Diagnóstico de crises não-epilépticas psicogênicas (CNEP), particularmente em pacientes com epilepsia, é de modo geral desafiador para os profissionais associados ao cuidado com estes pacientes. As desordens psiquiátricas são mais freqüentes em pacientes com epilepsia quando comparadas à população em geral, sendo ainda mais significativas em pacientes portadores de CNEP. Depressão e desordens de humor, bem como transtorno de ansiedade - particularmente os ataques de pânico - podem imputar dificuldades significativas ao manejo destes pacientes. Comorbidades psiquiátricas têm sido associadas a prognóstico mais reservado em pacientes portadores de CNEP. Intervenções psiquiátricas e psicológicas potencialmente podem melhorar o prognóstico, mas os estudos disponíveis são conflitantes. Detalhes e implicações práticas relacionadas a este cenário são apresentadas e discutidas.

The diagnosis of psychogenic nonepileptic seizures (PNES), particularly in patients with epilepsy, poses a special challenge to the physician in care of these patients. Psychiatric disorders (PD) are more common among patients with epilepsy than in the general population, and this appears to be even more important in patients with PNES. Depression and other mood disorders, as well as anxiety disorders - particularly panic attacks - may make the management of these patients even more difficult in the clinical practice. Concomitant psychiatric conditions have been associated with a poor outcome in patients with PNES]. Psychiatric and psychological intervention has been shown to be associated with improved outcome in PNES in outcome studies, although data is conflicting in this matter. The intricacies and practical implications of such issues are discussed.

OBJETIVOS: Foi avaliada a freqüência de transtornos psiquiátricos (TP) em uma série homogênea de pacientes com epilepsia do lobo temporal (ELT) associada à esclerose mesial temporal (EMT), com o objetivo de verificar a prevalência e os tipos de TP nessa população e de correlacionar tais transtornos a aspectos clínicos e à lateralidade da EMT. METODOLOGIA: Dados de 106 pacientes com ELT refratária associada a EMT foram revisados. A avaliação psiquiátrica foi baseada nos critérios diagnósticos do DSM-IV. A análise estatística foi realizada através do teste do qui-quadrado (chi²), do teste t de Student e do teste exato de Fisher. O valor de P considerado significante foi < 0,05. RESULTADOS: TP foram diagnosticados em 65 pacientes (61,3%). Transtornos do humor foram os mais freqüentemente encontrados (32 pacientes; 30%), seguidos por psicose interictal (15 pacientes; 14%) e por psicose pós-ictal (10 pacientes; 9.4%). Tanto a psicose interictal como a psicose pós-ictal foram associadas a EMT à esquerda (p < 0,05), enquanto TP em geral e transtornos do humor não foram correlacionados a nenhum lado. CONCLUSÃO: HOUVE uma elevada prevalência de TP em pacientes com ELT refratária associada a EMT. Os mais freqüentemente encontrados foram transtornos do humor e psicóticos. Quadros psicóticos foram associados a EMT à esquerda. Tais achados são concordantes com dados da literatura atual em relação à prevalência de TP na ELT, confirmando a existência de alterações anatômicas e uma possível associação da EMT à esquerda com quadros psicóticos na ELT.

OBJECTIVE: Behavioral changes in patients with epilepsy can range from depression, anxiety to psychosis and personality traits. We evaluated the frequency of psychiatric disorders (PD) in a homogenous series of patients with refractory temporal lobe epilepsy (TLE) related to mesial temporal sclerosis (MTS) aiming at determining the frequency of PD and possible correlations to clinical variables and to laterality of MTS. METHODS: Data from 106 refractory TLE patients were reviewed. Psychiatric evaluation was based on DSM-IV criteria. Statistical analysis was performed through the chi-square (chi²), Student's t test and Fisher's exact test. P value considered significant was < 0.05. RESULTS: PD were found in 65 patients (61.3%). Among them, mood disorders were the most frequent (32 patients; 30%), followed by interictal (15 patients; 14%) and postictal (10 patients; 9.4%) psychosis. Postictal and interictal psychosis were significantly associated with left side MTS (p < 0.05), while PD in general and mood disorders were not associated to any side. CONCLUSION: There was a high prevalence of PD in patients with refractory TLE associated to MTS. The most common were mood and psychotic disorders. Psychosis was significantly associated to left side. These findings are concordant with data in literature, confirming the existence of anatomic alterations, and also a possible left laterality effect in the mesial temporal lobe structures in patients with epileptic psychosis.

INTRODUÇÃO: A possibilidade cirúrgica é amplamente reconhecida como uma modalidade de tratamento no caso da presença de crises epilépticas refratárias à terapêutica medicamentosa. O objetivo maior da cirurgia é a redução ou eliminação completa das crises, sem comprometer significativamente o funcionamento cognitivo, além de melhorar a qualidade de vida (QV). OBJETIVO: Avaliar a QV em uma amostra de indivíduos com epilepsia do lobo temporal com esclerose do hipocampo antes e seis meses após o procedimento cirúrgico. METODOLOGIA: Trinta e cinco pacientes submetidos à corticoamigdalo-hipocampectomia e que consentiram em participar do estudo submetido e aprovado pelo Comitê de Etica da UNIFESP fizeram parte desta série. O ESI-55 foi respondido antes e seis meses após o procedimento cirúrgico. A análise estatística constou da análise da variância (ANOVA) e do efeito de tamanho da amostra para determinar as mudanças ocorridas na qualidade de vida após a intervenção cirúrgica. RESULTADOS: Seis meses após o tratamento cirúrgico 18 (51%) pacientes ficaram sem crises, 10 (29%) tiveram auras esporádicas e sete (20%) continuaram apresentando crises pouco freqüentes. A média dos escores do ESI-55 foi maior nos três grupos (sem crises, auras e crises), em todos os domínios após a cirurgia, com exceção das subescalas Funcionamento Cognitivo e Limitação por Problemas Cognitivos no grupo com crises. Uma melhora estatísticamente significante após a cirurgia foi observada nos domínios Estado Geral de Saúde (1.24), Bem-estar Emocional (1.32) e Energia/Fadiga (1.43). Conclusão: Embora aos seis meses já sejam verificadas algumas mudanças na qualidade de vida, avaliações periódicas anuais são fundamentais para o entendimento da extensão das modificações na QV após a cirurgia.

INTRODUCTION: Surgery is widely accepted as a modality of treatment for medically intractable seizures and the main goals of surgery as reduction or elimination of seizures without significant compromise in neurological function and improving QOL. PURPOSE: To assess QOL pre- and post-surgery in a sample of patients with temporal lobe epilepsy (TLE) due to mesial temporal sclerosis (MTS). METHODS: Thirty-five consenting subjects who had undergone corticoamygdalo-hipocampetomy were included in this study. They answered the ESI-55 before and six months after surgical treatment. Analyses of Variance (ANOVA) was used to study the changes in QOL and we also determined the effect-size of the sample. RESULTS: After surgical treatment 18 (51%) subjects were seizure free, 10 (29%) had only auras and seven (20%) had partial seizures. The post-operative mean scores were higher than pre-operative scores, except for Cognitive Function and Limitation due to Cognitive Problems in the group with seizures. A significant improvement in QOL post-surgery was observed in the domains Health-Perception (1.24), Emotional-Well-being (1.32) and Energy/Fatigue (1.43). CONCLUSION: A long-term follow-up is necessary to identify meanigful changes after the surgery.

INTRODUÇÃO: Auras somatossensoriais (AS) são mais frequentemente descritas em pacientes com epilepsia de lobo parietal (ELP), produzindo sensação de formigamento cutâneo envolvendo mão e dedos seguida de sinais motores focais tônicos ou clônicos. Estas crises habitualmente originam-se no hemisfério cerebral contralateral. Displasias, tumores, patologias vasculares ou gliose pós encefalite constituem as etiologias mais prováveis. Outras epilepsias focais, como as de lobo frontal e temporal podem também originar (AS). Embora este tipo de aura seja considerada rara em pacientes com epilepsia mesial do lobo temporal (EMLT), esta associação ainda não foi sistematicamente estudada. OBJETIVOS: A proposta deste artigo foi descrever quatro casos de EMLT refratária ao tratamento clínico, apresentando AS, reportar as características clínicas comuns, achados de neuroimagem e estudar o valor localizatório e lateralizatório das mesmas. MÉTODOS E RESULTADOS: Quatro pacientes com EMLT refratária e AS acompanhados no ambulatório de Epilepsia, Universidade Federal de São Paulo, foram submetidos a avaliação pré-cirúrgica. Sensação de formigamento envolvendo a extremidade superior foi o sintoma predominante em todos. Em três (75%) as auras foram contralaterais à esclerose mesial temporal (EMT). Em todos as AS eram, com maior freqüência, seguidas por outras auras autonômicas e psiquícas e evoluiam para crises parciais complexas (crises psicomotoras). CONCLUSÃO: Embora raras, AS podem estar presentes em EMLT. A associação de auras autonômicas e psíquicas, presença de crises parciais complexas (psicomotoras), déficits neuropsicológicos distintos, além de achados neurofisiológicos e de neuroimagem podem diferenciar pacientes com EMLT daqueles com ELP.

INTRODUCTION: Somatosensory auras (SSAs) are more usually described in patients with parietal lobe epilepsy (PLE), producing more commonly a localized cutaneous tingling sensation, involving hands and fingers followed by tonic or clonic focal seizures. These usually originate in the contralateral hemisphere. Etiology includes dysplasias, tumours, ischemic or postencephalitic gliosis. However, other focal epilepsies, such as frontal and temporal, may also originate SSAs. Although this type of aura is reported as rare in patients with mesial temporal lobe epilepsy (MTLE), this association has not been systematically studied. OBJECTIVES: The aim of this article was to describe the cases of four patients with refractory MTLE and SSAs, reporting their clinical characteristics and MRI findings. We discuss the localizing and lateralizing value of SSAs, particularly in the context of MTLE. METHODS AND RESULTS: Four patients with refractory MTLE and SSAs followed-up in the outpatient's clinic at the Epilepsy Section, Universidade Federal de São Paulo, were submitted to presurgical evaluation and corticoamygdalohippocampectomy. MRI in all cases showed unilateral mesial temporal sclerosis (MTS). Regarding seizure semiology, tingling sensation involving the upper extremity was the most prevalent symptom. Three of the four patients had SSAs contralateral to the MTS. Following the SSAs all patients most of the time presented other symptoms such as autonomic or psychic auras evolving to psychomotor seizures. After surgical treatment, two of the patients presented infrequent auras, and two were rendered seizure-free. CONCLUSION: Although rare, SSAs can be present in MTLE. The characteristics of autonomic or psychic auras, psychomotor seizures, neuropsychological deficits, and typical neurophysiologic and MRI findings may help differentiate patients with MTLE from those with PLE.

INTRODUÇÃO: O medo ictal (IF) é uma aura comum. Pode ser a unica manifestação de uma crise parcial simples ou o início de uma crise parcial complexa. Freqüentemente vivenciada por pacientes com epilepsia do lobo temporal mesial (TLE) é associada a comprometimento de estruturas mesiais como a amígdala. Os transtornos de ansiedade são muito comuns entre os transtornos psiquiátricos associados a epilepsia, com uma prevalência de 15 a 20%. OBJETIVOS: descrever três casos de pacientes com medo ictal como manifestação de epilepsia refratária do lobo temporal mesial, apresentando também resultados do EEG, exames de imagem, avaliação neuropsicológica, de qualidade de vida e psiquiátrica. MÉTODOS E RESULTADOS: Três pacientes acompanhados no ambulatório do setor de Epilepsia da Universidade Federal de São Paulo com epilepsia refratária do lobo temporal mesial foram submetidos a avaliação pré-cirúrgica e cortico-amidalahipocampectomia. Dois apresentaram EEG de superficie com eletrodos esfenoidais com início ictal do lado esquerdo e dois tinham transtorno de depressão maior segundo a avaliação psiquiátrica. CONCLUSÃO: Embora o medo ictal seja uma aura freqüente, é importante levar em conta os índices elevados de comorbidades psiquiátricas, como os transtornos ansiosos, nos pacientes com epilepsia. Concluiu-se que pode ser difícil o diagnóstico diferencial de transtornos de ansiedade interictal, ataques de pânico e medo ictal, exigindo um manejo cuidadoso.

INTRODUCTION: Ictal fear (IF) is one of the most frequent emotional auras. It is the sole or predominant manifestation of simple partial seizures or initial expression of a complex partial seizure. It is more often experienced in patients with mesial temporal lobe epilepsy (TLE), probably associated with mesial temporal structures, like the amygdala. Anxiety disorders are very common psychiatric disorders associated with epilepsy, with a prevalence of 15 to 25%. OBJECTIVES: To describe three patients with IF with refractory mesial TLE, also presenting the results of EEG, imaging exams, neuropsychological, quality of life and psychiatric evaluations. METHODS AND RESULTS: Three case reports of patients with refractory mesial TLE and IF followed up in the outpatient's clinic at the Epilepsy Section, Universidade Federal de São Paulo, Brazil, were submitted to presurgical evaluation and corticoamygdalohippocampectomy. Two patients presented ictal scalp-sphenoidal EEG onset on left side. Two patients had major depressive disorder in psychiatric evaluation. CONCLUSION: Although IF is the most frequent ictal psychological symptom, anxiety and mood disorders are very common psychiatric comorbidity in patients with epilepsy. It was concluded that differential diagnosis of interictal anxiety disorders, panic attacks and IF can be difficult, and requires careful management.

OBJETIVOS: Comparar os resultados em uma prova cognitiva de pacientes com epilepsia com dois parâmetros: um fornecido pelas normas brasileiras e outro pelas norte-americanas, tendo em vista as diferenças culturais. MÉTODO: Quarenta e cinco pacientes adultos com diagnóstico de epilepsia foram submetidos à avaliação de nível intelectual por meio da Escala Wechsler de Inteligência, WAIS-III nos seguintes itens: Quociente de Inteligência Verbal (QI V), Quociente de Inteligência de Execução (QI E) e Quociente de Inteligência Total (QI T). As correções dos protocolos seguiram as normas brasileiras e as norte-americanas. RESULTADOS: Dos 45 pacientes, 30 tinham o diagnóstico de epilepsia do lobo temporal (ELT) por esclerose mesial temporal (EMT), 15 no hemisfério esquerdo e 15 no direito, e 15 de epilepsia mioclônica juvenil (EMJ). Em todos os 45 protocolos do WAIS-III os resultados de QI V, QI E e QI T foram maiores quando empregados os parâmetros brasileiros. Assim, 18 pacientes que alcançaram QI T nas faixas "Média" e "Média Inferior", segundo as normas brasileiras, obtiveram níveis inferiores quando corrigidos pelas normas norte-americanas, ou seja, o QI T destes mesmos pacientes caiu para as faixas "Limítrofe" e "Muito Rebaixado" respectivamente. Nos pacientes com ELT, as diferenças de pontos entre os QI V e QI E não foram concordantes quando cotejados por normas diferentes e em cinco deles o desempenho verbal foi maior do que o motor quando corrigidos pelas normas norte-americanas. Nos pacientes com EMJ, os valores do QI V e QI E foram maiores quando empregadas normas brasileiras. CONCLUSÃO: Os resultados da avaliação cognitiva pelo WAIS-III de pacientes com epilepsia mostram que a interpretação do nível intelectual é muito distinta quando corrigida segundo normas brasileiras ou norte-americanas. Há também diferença na análise qualitativa do desempenho intelectual, uma vez que o estudo das discrepâncias internas variou conforme a norma populacional utilizada.

OBJECTIVES: To compare the results of a cognitive task of patients with epilepsy with two parameters: one provided by the Brazilian and the other by the US normative scores viewing the cultural differences. METHOD: Forty-five adult patients diagnosed with epilepsy were submitted to intellectual evaluation through the Wechsler Adult Intelligence Scale, WAIS-III, in the following items: Verbal Intelligence Quotient (V IQ), Performance Intelligence Quotient (P IQ) and Full Scale (FS IQ). The correction of the protocols followed the Brazilian and the US normative scores. RESULTS: Thirty out of the 45 patients were diagnosed with temporal lobe epilepsy (TLE) due to mesial temporal sclerosis (MTS), 15 in the left hemisphere and 15 in the right, and 15 with juvenile myoclonic epilepsy (JME). In all 45 WAIS protocols the results of the V IQ, P IQ and FS IQ were higher when the Brazilian parameters were employed. Therefore, 18 patients that achieved FS IQ in the "Average" and "Low Average" ranges, according to the Brazilian normative scores, got lower levels when corrected by the US normative scores, that is, the FS IQ of these 18 patients fell to the ‘Borderline’ and ‘Extremely Low’ ranges respectively. In the patients with TLE, the differences in the scores between the V IQ and P IQ did not correspond when evaluated by different normative scores and in five of them the verbal was higher than the performance when corrected by the US normative scores. In the patients with JME, the values of V IQ and P IQ were higher when the Brazilian normative scores were employed. CONCLUSION: The results of the cognitive evaluation through the WAIS of patients with epilepsy showed that interpretation of the intellectual level is very dissimilar when corrected by the Brazilian or US normative scores. There is also a difference in the qualitative analysis of the intellectual performance since the study of the inner discrepancy varied according the population normative scores employed.

OBJETIVOS: Fusos Extremos (FE) foram descritos por Gibbs e Gibbs em 1962. São tipicamente observados em crianças menores de cinco anos, ocorrendo em 0,05% de crianças normais e em 5 a 18% das crianças com retardo mental ou paralisia cerebral. Nesse estudo, descrevemos as características clínicas e neurofisiológicas de 5 pacientes com FE, correlacionando esses achados com os aspectos de neuroimagem, através de exame de ressonância magnética (RM) de encéfalo. PACIENTES E MÉTODOS: Foram selecionados inicialmente oito pacientes do ambulatório de epilepsia infantil da UNIFESP, que apresentavam fusos extremos em pelo me-nos um exame de eletroencefalograma (EEG). Dos oito, foram selecionados cinco (três do sexo feminino) que tinham RM de encéfalo disponível para análise. Foram verificados as características clínicas, o desenvolvimento neuropsicomotor e os achados neurofisiológicos, bem como os achados da RM de encéfalo. RESULTADOS: A idade dos pacientes variou de 2 a 16 anos. Os cinco pacientes apresentavam déficits cognitivos, e três apresentavam déficits motores associados. Todas as crianças tinham diagnóstico de epilepsia, sendo que em três as crises estavam controladas com drogas antiepilépticas (DAE) enquanto em dois, eram refratárias ao tratamento clínico. Em apenas um paciente a RM foi normal. Nos demais, os achados foram: paquigiria bilateral, atrofia cortical difusa, lesão focal occipital direita e atrofia frontal bilateral. A freqüência dos FE variou de 8,9 a 16 Hz, a amplitude de 67 a 256 µV. Em três pacientes, além dos FE foi observada atividade rápida de localização frontal. CONCLUSÃO: FE são raramente observados em crianças normais, mas podem ocorrer com maior freqüência naquelas com retardo mental ou paralisia cerebral. FE não parecem estar associados a epilepsia, mas em nossa série todos as crianças tinham diagnóstico de epilepsia, bem como retardo mental. O estudo com RM mostrou que FE podem ocorrer em crianças com anormalidades estruturais definidas bem como naquelas com neuroimagem normal, sugerindo que esse padrão se associa à retardo mental, mas não a etiologias específicas.

OBJECTIVES: Extreme spindles were described by Gibbs and Gibbs in 1962. They are typically observed in children younger than five years, occurring in 0.05% of normal children and in up to 5 to 18% of children with mental retardation or cerebral palsy. In this study we describe the clinical and neurophysiological characteristics of five children with extreme spindles, correlating these findings to neuroimaging data, obtained in magnetic resonance imaging of the brain. PATIENTS AND METHODS: Eight patients from the children epilepsy outpatient clinic at UNIFESP were initially included, who had extreme spindles in at least one electroencephalogram (EEG) examination. Five out of these eight were selected, since they had MRI of the brain available for analysis. RESULTS: The age of the children varied from two to 15 years. The five children had mental retardation, and three presented associated motor deficits. All had epilepsy; in three children seizures were controlled with antiepileptic drugs, but in two they were considered refractory to medical treatment. In one patient only the MRI of the brain was considered normal. In the other cases, the findings were: bilateral pachygiria, diffuse brain atrophy, right occipital lesion, and bilateral frontal atrophy. The frequency of extreme spindles varied form 8.9 to 16 Hz, and amplitude from 67 to 256 µV. In three patients, frontal fast activity was observed along with extreme spindles. CONCLUSIONS: Extreme spindles are seldom observed in normal children, but may be quite frequent among those with mental retardation or cerebral palsy. They are probably not related to epilepsy, though in our series all children had epilepsy as well as mental retardation. Diagnostic investigation of these children with MRI of the brain showed that extreme spindles may occur either in children with defined structural abnormalities or in those with normal neuroimaging examination, suggesting that this specific electroencephalographic pattern is associated to mental retardation but not to specific etiologies.

Introdução: Dificuldades no trabalho, em relacionamentos interpessoais, familiares e sociais, a percepção do estigma, da discriminação dentre outros têm sido associados aos estados depressivos interictais, influenciando negativamente a qualidade de vida (QV) das pessoas com epilepsia. A depressão tem alta prevalência (20 a 55%) nas epilepsias, sendo vista como o fator mais importante no julgamento do paciente sobre sua QV. Objetivos: Avaliar a ocorrência de sintomas depressivos em pacientes com epilepsia do lobo temporal por esclerose mesial temporal (EMT); estudar a associação entre sintoma depressivo e QV; e entre localização da lesão estrutural e sintomas depressivos. Metodologia: Setenta pacientes com EMT em tratamento ambulatorial, foram avaliados entre junho/2003 e abril/2005. Além da anamnese clínica, exames subsidiários (EEG, vídeo-EEG, RNM), avaliação neuropsicológica, psiquiátrica, todos foram submetidos à avaliação de QV que incluiu uma entrevista semidirigida, o questionário Medical Outcomes Short-Form 36 (SF-36) e o Inventário de Depressão de Beck (BDI). Resultados: Quarenta e dois pacientes (60%) eram do sexo feminino e 28 (40%) do masculino, com média de idade de 37 anos e 26 de duração da epilepsia. Em quarenta (57%) foi constatada EMT à esquerda e em 30 (43%) à direita. Quanto à situação de trabalho, 34 (49%) encontravam-se em auxílio doença/aposentados por invalidez/ou sem qualquer atividade produtiva. Trinta e um (44%) alegaram ser o trabalho a esfera mais prejudicada pela epilepsia. Trinta e nove indivíduos (56%) apresentaram sintomas depressivos com escores mais baixos no SF-36 quando comparados àqueles sem depressão. A pontuação alta do BDI, indicativa de depressão manteve uma associação significante (coeficiente de Spearman: p < 0,001) com todos os domínios do SF-36, com exceção de capacidade funcional (CF). Não houve correlação entre o lado da EMT e os sintomas depressivos. Conclusão: A presença de depressão infuencia o julgamento individual em relação a QV. O uso sistemático de instrumentos de fácil manuseio permitiria a identificação precoce de distúrbios de humor, contribuindo significativamente para a melhoria da QV de indivíduos com epilepsia.

INTRODUCTION: Depressive symptoms and quality of life in people with epilepsy related to mesial temporal sclerosis Introduction: Difficulties in working, interpersonal, familial and social relationships, the perception of stigma, the discrimination by others have been associated with interictal depressive state influencing negatively the quality of live (QOL) of people with epilepsy. Depression has a high prevalence (20-55%) in epilepsies, being considered the most important factor in the judgment of the patient concerning his QOL. Objective: To evaluate the occurrence of depressive symptoms in patients with temporal lobe epilepsy due to mesial temporal sclerosis (MTS); to study the association between depressive symptoms and QOL as well as of the lateralization of the structural lesion and depressive symptoms. Methodology: 70 patients with MTS in the Outpatient Clinic were evaluated between June 2003 and April 2005. Besides the clinical anamnesis and examination all the patients had ancillary exams (EEG, video-EEG, MRI), psychological and psychiatric evaluation and QOL evaluation which was performed with a semi-structured interview, the questionnaire Medical Outcomes Short Form 36 (SF-36) and the Beck Depression Inventory (BDI). Results: 42 (60%) patients were female and 28 (40%) male. The average age was 37 and the mean epilepsy duration 26. In 40 (57%) MTS was located on the left and in 30 (42%) on the right. As to the work situation, 34 (49%) were in welfare/retired due to illness/without any productive activity. 31 (44%) stated that lack of work was the greatest burden of epilepsy. 39 (56%) presented depressive symptoms with lower scores in the SF-36 when compared to those without depression. A high score of the BDI, an indication of depression, maintained a significant association (Spearman coefficient p < 0.001) in all SF-36 domains with the exception of Functional Capacity. There was, however, no correlation between the lateralization of MTS and depressive symptoms. Conclusion: The presence of depression influences the personal judgment in relation to QOL. The systematic use of simple instruments would allow the precocious identification of humor disturbances contributing significantly to the improvement of QOL of people with epilepsy.
